REFUND GUARANTEE FORM

In the event that the company with details listed below (“Client”) fails to receive approval for their Productivity
Solutions Grant (PSG) application for the purchase of HRM Labs Shared Service:

1. The client shall be eligible to request and receive a full refund from HRM LABS PTE. LTD (“HRM Labs”) on
any client payments made towards the purchase of Services, upon the Client’s request; and
2. HRM Labs shall not commence the provision of Services that have been refunded.

Refunds will not be made out for any other reason aside from the above.
